What Is A Concussion?

Concussions are the most common type of traumatic brain injury and millions occur each year. These injuries result from a sudden blow or jolt to the head, disrupting normal brain function. These may also occur without a hit to the head from a rapid movement such as seen with a whiplash injury. 

Concussions can occur in a variety of situations, such as sports injuries, car accidents, or falls. While they might initially seem minor because imaging is routinely normal. These injuries can have significant short- and long-term impacts on your health if not properly addressed. The symptoms of a concussion can vary widely, including headaches, dizziness, memory loss, and difficulty concentrating.

What Happens to the Brain During a Concussion?

When the brain experiences a sudden impact, it can shift or twist within the skull, leading to cellular damage and chemical imbalances. This disruption interferes with how brain cells communicate and can cause inflammation. In severe cases, the injury may even lead to structural changes in the brain that can be seen using a specialized MRI known as a DTI scan.

 

There are 4 big things that happen in every brain following a concussion:

  • Impaired Connectivity
  • Mitochondrial Dysfunction
  • Immune Changes (Inflammation)
  • Blood Flow and Oxygenation

These changes are what contributes to the initial clinical signs and symptoms, as well as persisting symptoms that may last for years. 

 

The initial symptoms of a concussion will typically manifest during the first 48 hours. These include headaches, nausea, dizziness, light sensitivity, mood changes, and blurry vision. Clinical signs such as confusion, loss of consciousness, vomiting, or issues with walking may occur; but this is not required to meet criteria to have suffered a concussion.

How Are Concussions Diagnosed and Treated?

Diagnosing a concussion involves a combination of patient history, symptom evaluation, and testing to evaluate neurological function. While conventional imaging, like CT and MRI scans, may not detect subtle brain injuries, there are advanced imaging modalities that may be beneficial. These include DTI, PET, fMRI, and SPECT scan. There isn’t one scan though that is perfect and picks up everything. Beyond that most of these imaging modalities are expensive and don’t quite change the treatment that would be performed at this time.

 

Best Testing For Concussion

The best testing for a concussion is to evaluate function. This includes eye movements, balance, cervical spine, and processing speed. Most individuals with a concussion will never have all of these evaluated. It is important to evaluate all of these and not just one as that is an incomplete exam. We evaluate at least 90 markers of brain function. 

Neurocognitive assessments are also important in identifying deficits in memory, attention, and processing speed. It should be noted that you can test normal with these tests and still have a concussion. We’ve had many patients where a neuropsychologist said they didn’t have a concussion due to these tests; this wouldn’t be appropriate.

Previously it was recommended to do nothing and let time pass for treatment. This has been rejected as it actually prolongs the time to recovery. If someone decides to use traditional approaches to concussion recovery they will often be prescribed medications to address symptoms and be sent to physical therapy for musculoskeletal related symptoms.

That treatment approach doesn’t address the underlying factors with concussion and many won’t get better. It’s been suggested in numerous research articles that 50% of individuals with a concussion are still struggling even a year later which is proof the treatment isn’t sufficient.
